OSHA standard 1910.23 establishes mandatory requirements for all fixed ladders in US general industry workplaces. Before you purchase or install a fixed ladder, you must know the dimensional, load, and fall protection requirements that apply to your specific installation. Non-compliance can result in OSHA citations, project delays, and — most importantly — worker safety risks. This article summarises the five key requirements that every procurement manager, facility engineer, and contractor must address.
Requirement 1: Dimensional Compliance (1910.23(d)(1)-(3))
Three dimensions define a compliant fixed ladder:

What to verify during site inspection: Measure the clearance from the wall to the nearest permanent obstruction behind the proposed ladder location. If building services (pipes, ducts, conduit) reduce clearance below 7 inches, the ladder must be offset or the services rerouted.
Requirement 2: Fall Protection — The 24-Foot Rule (1910.23(d)(4))
This is the most critical — and most misunderstood — OSHA requirement for fixed ladders.
The basic rule: Any fixed ladder extending more than 24 feet (7.3m) above a lower level must have fall protection.
The 2018 change: For ladders installed after November 19, 2018, the fall protection must be a ladder safety system or personal fall arrest system (PFAS) — a cage alone is not sufficient.
The 2036 deadline: All existing fixed ladders over 24 feet installed before November 19, 2018, must be retrofitted with a ladder safety system or PFAS by November 19, 2036.

Requirement 3: Top Extension Above Landing (1910.23(d)(4)(iv))
The ladder’s side rails must extend at least 42 inches (1,100mm) above the top landing surface. This extension provides a handhold for the climber to safely transition from the ladder to the roof, platform, or walkway.
Common violation: Ladders terminating flush with the roof edge or platform. Without the 42-inch extension, the climber has nothing to hold onto during the most critical part of the climb — the transition.
Dengtai standard: All ladders are manufactured with a 1,100mm top extension as standard. If you need a different extension height (e.g., for a parapet step-through configuration), specify this at the time of order.
Requirement 4: Load Capacity (1910.23(l)(1))
Each ladder must be capable of supporting, without failure, its maximum intended load.

Requirement 5: Inspection and Maintenance
OSHA 1910.23(b)(9) requires that ladders be inspected before initial use and on a periodic basis.
- Frequency: No specified interval in the standard — the employer must determine the appropriate frequency based on use and environment. Annual inspection is industry standard for outdoor installations.
- What to inspect: Rung condition, stile straightness, bracket tightness, cage integrity, corrosion, anchor security, and top extension condition.
- Documentation: Inspection records must be maintained. A tag or label system indicating the last inspection date and pass/fail status is recommended.

Frequently Asked Questions
1. Does OSHA 1910.23 apply to all US workplaces?
OSHA 1910.23 applies to general industry workplaces (manufacturing, warehousing, utilities, commercial facilities). Construction sites fall under OSHA 1926 Subpart X, which has different requirements for portable ladders but similar requirements for fixed ladders. Maritime and agricultural operations have their own specific standards. If you are unsure which standard applies to your facility, consult a qualified safety professional — applying the wrong standard can result in non-compliance even if the ladder meets the requirements of the standard you used.
2. What happens if my ladder is inspected after November 2036 and still only has a cage (no safety system)?
OSHA can issue citations for non-compliance. The 2036 deadline is firm: all existing fixed ladders over 24 ft that were installed before November 19, 2018, must have a ladder safety system or PFAS by that date. The citation can be classified as “serious” (up to $15,625 per violation) or “willful” (up to $156,259 per violation) if the employer was aware of the requirement and did not act. If you have ladders approaching the 2036 deadline, plan the retrofit now — waiting until 2035 risks supply chain delays, contractor availability issues, and potential citation if OSHA inspects early.
